Medical Records

Your medical records can be obtained via the following methods:

• Log into your Patient Portal. Visit notes, lab results, and immunization records can be easily printed directly from your portal. 

• ​Call the office and we can text you the Medical Records Release form. Once we received the signed copy, we will process your records accordingly. 

There may be a charge for medical records that need to be printed or faxed.
