Medical Records

Your medical records can be obtained via the following methods:

• Log into your Patient Portal. Visit notes, lab results, and immunization records can be easily printed directly from your portal. 

• ​Print & fill out the Medical Records Form. You can mail, fax, or bring the form by in person. 

There may be a charge for medical records that need to be printed or faxed.
